6.  Can I fax forms to the Fund?
Generally, if you are making an official request for a form or information about your account, the Fund will accept a fax. In some cases, we need the original signed form to be sent by mail. In other cases, like a Designation of Beneficiary form, we can only accept an original, signed, and notarized form. Be sure to check with the Fund to find out which forms can be faxed.
